OPENING HOURS QUESTIONNAIRE - This questionnaire has been design in collaboration with our Patient Participation Group.

We would like you to take a few moments to complete this (see link below) so we can obtain your views on how you really feel about the services/access we offer.

To ensure that you can have the drop down menu please close the box at the bottom of the screen. Once you have completed the questionnaire (you will have to save it on your computer) then please 'send to' michelle.armstrong@gp-p83020.nhs.uk
