Sick Notes

If you are off work with an acute illness:

• After 3 days absence you will need to do a self-certification note, which is available from your employer, the post office or the reception at Minden.
• Please note that you only require a sick note from your doctor after 7 continuous days' absence.

We do not provide doctors' notes for periods less than 7 days.

Patients requesting appointments purely for sick notes for before seven days will obviously make it more difficult for other patients to get appointments.

If you off work for more than 7 days then you need to see your doctor to obtain a sick note.

Firms cannot and should not demand doctor's sick notes for less than seven days. If they do there will be at least a £10.00 fee.
