Register with a Doctor
If you wish to register with a doctor at Minden please check that you live within the practice area and then come to the centre to see the receptionist.
It is very helpful if you can bring your medical card with you. Do not worry if you haven't got it as you can complete a separate form. The information needed is (this is to help the NHS trace your previous records)
- Your name, date of birth and NHS number (if you know it).
- Your town and country of birth.
- Your previous address.
- Your previous doctors name and address.
- If you are from abroad your first UK address where you registered with a GP & the date you first came to live in the UK.
- If you were previously resident in the UK your date of leaving.
- If you returning from the Armed Forces your address before enlisting, your Service or Personnel No and your enlistment date.
We shall also ask you to complete some medical information forms detailing your medical history.

If you wish to register with a particular doctor please approach the receptionist for that doctor. If you do not have a particular preference then we have a rota for new patient requests.
After you have handed in your application please phone that doctors receptionist two days later to see if your application has been accepted.
We are able to accept most requests from patients moving into the practice area. However due to workload levels we are not often in a position to accept requests from patients who all ready have a doctor in the Bury area, unless there are exceptional circumstances at the discretion of the doctor.
If your application is accepted please make an appointment with the doctor or nurse for a new patient check. This allows us to check a few things like your blood pressure, to make sure we have a picture of your previous medical history and also just to say hello! Please bring a urine sample with you.
