District Nursing

The district nursing team have recently been relocated to Inwood House you can contact them on 0845 313 4105. They still call in to Minden 3 - 4 times a week to pick up any messages that have been left for them.

The District nursing teams offer a unique service to the community of individuals registered with the practice. The District Nurses have a generic role.

The service offers a holistic approach to the care of individuals to make choices about their health and well being. The team offers support and guidance not only to these individuals but also to their families and careers.

Treatments such as dressings, injections and venepuncture are done at home if for whatever reason the patient cannot attend the treatment room.

District Nurses also care for individuals suffering from long term illness, helping them to die at home in peace, comfort and dignity. The team also do, bereavement visits to relatives, although this is not a formal counselling session it offers a supporter network if needed.

The team philosophy is, all disciplines will work together to identify needs of the practice population and take appropriate action to help people achieve their optimum level of health. They will provide patients with the information assistance and care suitable to the individual's need's, liaise with other agencies and operate in accordance with local and national guidelines.

The Nurses take patient referrals from many sources GP's, hospitals, Social services but to name but a few.

We have been part of an integrated nursing team since July 1998. Taking responsibility for our management and budgetary control on a day to day basis. Having integrated team meetings with other members of the multidisciplinary team to improve communication and collaboration between individual disciplines. These are held monthly.

We have regular meetings with our GPs. to discuss individual patients, and the care that we are giving, and update them on their progress.

We work very closely with all specialist nurses from the trust to enhance the care given to all our patients.

Within the team we have individuals, who have particular areas of interest in, eg. diabetes, Palliative care, leg ulcer management, continence management, smoking cessation and wound tissue viability, these skills help the other members to deliver a high standard of care to our patients.
